You'll have to play around with it a bit to get the hang of it, but you use it to reshape the clothing item to fit your original avatar.
Big hint for using it, it starts out zoomed way huge, scroll mouse wheel down to get the circle to something sane. If it's just dragging your whole model around the screen, it's too big.

The only downside is, it doesn't actually come with a texture file, instead the colors are all stored via vertex colors, and .obj doesn't seem to work with vertex colors. It begs the question as to why the hell .obj is the format you download it in to begin with, but whatever. Actually another downside is that the amount of polys is god damn huge. 100k+ polys, if not sometimes 200k+. If you download a tool called Meshlab, convert it into a .ply, export it back into blender, create a new material, scroll down and check "Vertex Color Paint" and "Vertex Color Light", and then setting the shading to Shadeless under MMD Shading on the tool panel to the right you should be able to get it working.
